BioMend Health provides comprehensive mobile primary care services designed to meet patients where they live. Our services mirror traditional primary care offerings while removing barriers associated with travel and clinic-based visits.
BioMend Health participates in Medicare Part B. Services are billed in accordance with Medicare guidelines. Patients are encouraged to contact our office for additional information regarding eligibility and coverage.
Mobile Primary Care
Routine medical evaluations, preventive care, acute illness assessment, and ongoing health monitoring provided directly in the patient’s residence.
Chronic Condition
Management
Proactive management of chronic and complex conditions with a focus on stability,
education, and quality of life.
Transition Care Management
Support following hospital or skilled nursing discharge to promote continuity, medication
safety, and recovery.
Wound & Skin Integrity Care
Ongoing assessment and treatment of wounds and skin conditions with attention to environmental and contributing factors.
Medication Management
Medication reconciliation, monitoring, and education to support safe and effective
treatment.
Care Coordination
Collaboration with caregivers, facilities, and community providers to ensure aligned and
coordinated care.
INSURANCE
BioMend Health accepts Medicare Part B for eligible mobile primary care services. Coverage is based on medical necessity and individual benefit plans. Our team is happy to assist patients and families with questions regarding coverage and billing.
